Part of our Aging Well series

Advance planning for end-of-life health care brings the peace of mind that comes with knowing that your family and health care providers have clear directions from you. Why not make it a New Year’s resolution to get your plans and wishes down in writing?

This practical online workshop will discuss completing an Advance Directive, choosing a Durable Power of Attorney for Health Care, Instructions for Artificial Feeding and Hydration, filling out POLST (Portable Orders for Life Sustaining Treatment) forms, completing directives for mental health and for dementia, and more.
